Panel Management Assistant

Who we are: La Clínica de La Raza is a community-based health center committed to providing culturally appropriate, high-quality, and accessible health care to the diverse communities of the San Francisco Bay Area. We have spent over 40 years advocating for and creating a health home for the many that have been denied access to care. As health care activists, we are dedicated to making sure individuals who do not have health coverage get the same level of quality healthcare as those who have it. From our genesis as a community health center in 1971, we understood that we cannot heal our communities without also addressing the economic and social factors that affect health. Many times, this requires that we go outside and provide services in other settings to build the bridge that links individuals to our health center for preventive and primary care. Over 90,000 individuals come to La Clínica because it is a welcoming place that addresses the whole person, coordinating and connecting them to a broad network of services to improve and maintain their health and well-being. While we are still known for our activism and spirit of social justice, we are also proud to have grown into a sophisticated provider of primary health care services with 35 sites across Alameda, Contra Costa and Solano counties.

Major Areas of Responsibility include but are not limited to:

Commitment to Quality

  • Bring creativity and inventiveness to the workplace to help design quality improvement initiatives and rally staff together for collaborative work
  • Lead your clinic team through PDSA (Plan-Do-Study-Act) cycles of quality improvement
  • Attend webinars and trainings on an ongoing basis to enhance knowledge and relay information to support quality improvement initiatives
  • Familiarize yourself with the language of quality improvement, such as: HEDIS measures, UDS, scrubbing, etc.
  • Participate and support in the spirit of collaboration with other Panel Managers across clinic sites through Community Calls and other events
  • Demonstrate a commitment to excellent patient experience and quality outcomes
  • Communicate and work effectively with providers and other members of the care team to best serve patients.
  • Contribute to a welcoming clinical space and healthy work environment through proactive relationship building, direct communication and sound judgment.
  • Engage participation in huddles, staff meetings, team building activities and assigned development opportunities.
  • Continue your professional development through participation in educational opportunities.
  • Adhere to all La Clínica ethical and professional standards, including La Clínica’s policies and procedures.

Panel Manager Assistant Duties

  • Receive ongoing training on report-building and data stewardship
  • Update and maintain patient registries using a robust tracking system that can track patients based on panels belonging to individual primary providers OR disease /condition.
  • Assist care members with running QI reports using available tools to aid in closing gaps in care, and models and techniques for improving care such as the Model for Improvement
  • Incorporate the patient perspective into workflows and QI techniques
  • Assist the care team to perform patient outreach and in-reach in order to close care gaps
  • Aid in creating dashboards for the display of agency and site-specific data
  • Provide input (specifications) for new reports that would help improve the quality of care
  • Assist in validating new QI reports
  • Review gaps in care lists from payers and internal sources for accuracy
  • Help manage reporting, interventions and logistics for grants and special initiatives. Review registries for target populations/panels, to conduct predetermined interventions agreed upon by health care team. This may include management of abnormal pap and mammography registries.
  • Conduct chronic disease-focused patient visits under protocol (see below)
  • Perform other duties as assigned

Patient Care

  • Verify purpose of visit with the patient.
  • Maintain strict confidentiality regarding patient information.
  • Administer relevant health questionnaires.
  • Prepare patients for examination by taking vital signs.
  • Effectively communicate requests from patients to provider or care team members.
  • Involve a higher level of care immediately if needed based on the visit protocol.
  • Coordinate patient flow to maximize patient access and service.
  • Explain and/or translate treatment procedures, medications, diets, or providers' instructions to patients.
  • Schedule follow-up appointment or recall appointment after visit.
  • Provide prescription information to pharmacies, as directed by a provider.
  • Complete and review information at end of patient visit.
  • Document in EHR and electronic tracking systems.
  • Ensure that all logs, forms, records, and charts are complete, thorough, and accurate.
  • Provide patient education and follow-up care instructions as instructed by provider.

Basic Knowledge Required

  • Working with modern computer applications and electronic health records.

Abilities

  • Bilingual proficiency in Spanish or (Specify Language). Use if recruiting for a language specific position only
  • Demonstrate flexibility and ability to accept changes gracefully.
  • Able to work professionally and collaboratively in team-based care delivery.
  • Demonstrate comfort with direct patient contact and solid interpersonal skills for patient education
  • Ability to work across cultures and demonstrate support of diversity, equity and inclusion.
  • Use clear and efficient written and verbal communication skills using the English language.
  • Complete and maintain accurate and concise patient records.

Minimum Qualifications:

Required:

  • High school diploma/equivalent.
  • One year of experience in a healthcare delivery setting, in a paid or volunteer capacity.
  • Must hold a current BLS (Basic Life Support) Certificate. You will have 30-day grace period to obtain your certificate from the date of hire.
  • Certification for health clearance required of all healthcare professionals

Preferred:

  • Types of healthcare experience preferred: medical assistant, clinical office assistant, health educator
  • Bachelor’s Degree from accredited University with an interest in healthcare
