Street Outreach Worker

Who we are:  La Clínica de La Raza is a community-based health center committed to providing culturally appropriate, high-quality, and accessible health care to the diverse communities of the San Francisco Bay Area.  We have spent over 40 years advocating for and creating a health home for the many that have been denied access to care.  As health care activists, we are dedicated to making sure individuals who do not have health coverage get the same level of quality healthcare as those who have it.   From our genesis as a community health center in 1971, we understood that we cannot heal our communities without also addressing the economic and social factors that affect health. Many times, this requires that we go outside and provide services in other settings to build the bridge that links individuals to our health center for preventive and primary care.  Over 90,000 individuals come to La Clínica because it is a welcoming place that addresses the whole person, coordinating and connecting them to a broad network of services to improve and maintain their health and well-being.  While we are still known for our activism and spirit of social justice, we are also proud to have grown into a sophisticated provider of primary health care services with 35 sites across Alameda, Contra Costa and Solano counties. 

Be on the frontlines of community outreach:  We are looking for a Street Outreach Worker (SOW) who is committed to giving our diverse community the high quality of service they deserve. SOW’s usually spend most of their time in community based settings such as bars and outdoor venues speaking to individuals who may need support and assistance. The individual in this position will conduct rapid HIV testing and provide referrals. To be successful, you need to enjoy interacting with people of diverse ethnic, economic and educational backgrounds as well as alcohol and other drug users, people of different sexual orientations and people living with HIV/AIDS.  The ideal candidate has a passion for service and a dedication to community. Major Areas of Responsibility include but are not limited to: 

Testing & Education

  • Conduct rapid HIV testing
  • Conduct pre and post- test HIV test counseling to identify underlying risk co-factors and risk reduction goals.
  • Collaborates and Network effectively with other community agencies that provide services to the target population.
  • Provides referral and follow-up referrals to YMSM of color

Records Maintenance

  • Completes and maintains accurate records of people served by our testing service in the Alameda County Data Entry System, Leo. 
  • Assists in creating CDC reports
  • Implementation of Evidence Based Intervention Empowerment/Healthy Relationships
  • Other duties as assigned by supervisor

Community Outreach & Coordination

  • Participates in health fairs, community events, and other events where the target population gathers
  • Coordination of the YMSM Advisory Board
  • Coordination of the recruitment of YMSM to participate in structure group setting using CDC EBI interventions

 

Minimum Job Requirements

Knowledge

  • Knowledge of referral system process
  • Knowledge of East Bay area preferred
  • Cultural competence working in diverse/low income communities

Abilities

  • Ability to perform work in community-based settings (bars, outdoors venues and testing van) may or may not be air conditioned and may result in exposure to second- hand smoke.
  • Ability to work with people of diverse ethnic, economic, and educational backgrounds, Alcohol and Other Drug Users (AOD), people of different sexual orientations, and people living with HIV/AIDS
  • Ability to work night hours and weekends as required to meet community needs.
  • Ability to stand 50% of the time includes walking around.
  • Ability to lift at least 10 lbs.
  • Must have a valid California driver’s license and reliable transportation.
  • Ability to organize and maintain records
  • Ability to speak Spanish fluently preferred

Experience and Other Certifications

  • High School diploma required or equivalent supplemented with three or four years’ experience working with young MSM of color in the areas of community health education, and/or community organizing and HIV counselor
  • Must hold a current BLS (Basic Life Support) Certificate. You will have 30-day grace period to obtain your certificate from the date of hire.
  • HIV Test Counselor certification required
  • Experience working independently as well as in a team; maintains good working relationships with staff and community members.
  • Experience facilitating small and large groups
